Thank you for attending the UUP Benefits Overview session. As discussed during this presentation, you have 28 days from your date of hire to complete these tasks/enrollments. Any delays may result in longer waiting periods or plan ineligibility.
After the Benefits Overview Session, you received an email with forms and enrollment instructions. Below is a review of what you received. Please use the corresponding submission buttons on the right of your screen to submit each of your enrollment forms.
Retirement Plan Election:
To enroll in a retirement plan you must create/log in to the SUNY Retirement at Work Portal.
Remember, when accessing Retirement at Work, you will need to select "Stony Brook University" as your employer. You will only indicate Stony Brook Medicine if agency code is 28058, this code will be listed on the top right corner of your paystub.
Please be advised, if you will be electing either the ERS or TRS pension plans, it is a two step process. You will make your electronic election through the SUNY Retirement at Work Portal and also submit the corresponding paper application. Please note, if you are electing either ERS or TRS, these paper forms require a physical signature, digital signatures are not acceptable. To submit your completed paper enrollment form, please use the "Pension Plan Enrollment" button on the right hand side of your screen.
Health Insurance Enrollment:
To enroll in the health insurance plan, please submit your completed form PS404 and all of the required proof documents. You will need to upload this form along with all of the proof documents. To upload, please use the "New Hire Health Insurance Enrollment" button on the right hand side of your screen.
Other Forms:
Please use the "New Employee Benefits Overview Checklist" button on the right hand side of your screen to submit your completed Orientation Check List.
