CELT Course Content Accessibility Requests

What Is It?

Request assistance with achieving accessible digital content through the Center for Excellence in Learning and Teaching (CELT).

This form is used to gather information to help schedule a consultation on the accessibility of digital course content (e.g., a file, a page, electronic message, or multimedia shared).

When your consultation is scheduled, please provide access to the file or page before the meeting.

Note, there are training opportunities available to support the long-term development of accessible digital content. If you have used these resources and are still having issues, please request a consultation by clicking the Request Assistance button.

If you are a student filling this out and have a physical, psychological, medical, or learning disability that may impact your course work, please contact the Student Accessibility Support Center, Stony Brook Union Suite 107, (631) 632-6748, or at sasc@stonybrook.edu. They will determine with you what accommodations are necessary and appropriate. All information and documentation is confidential.
