Class of 2030 Attestation Form

Please review and affirm the School of Dental Medicine (SDM) Policies & Procedures, Honor Code, and Technical Standards that are included in the Student Handbook and are available to students via the website. These policies will also be reviewed each year during orientation.  

I acknowledge that I have reviewed, understand, and agree to abide by the SDM Academic Policies and Procedures. I attest that these policies and procedures govern my conduct, responsibilities, and progression within the School of Dental Medicine, and I accept full responsibility for adhering to them throughout my enrollment.

I hereby affirm my acceptance of the SDM Honor Code. I commit to upholding the highest standards of honesty, integrity, professionalism, and ethical conduct in all academic and clinical activities. I acknowledge my responsibility to maintain both personal and collective accountability and to contribute to an environment built on trust, respect, and professionalism among peers, faculty, staff, and patients.

I acknowledge that I have reviewed and understand the Technical Standards for Admissions and Progression for the School of Dental Medicine. I attest that I am able to meet these standards, with or without reasonable accommodations, and that I accept responsibility for maintaining the physical, communication, cognitive behavioral, and professional competencies required for successful participation in all academic, laboratory, pre-clinical, and clinical activities. I further recognize that continued compliance with these technical standards is essential for progression and graduation from the program.
